How to Read Piano Music
Although its tedious, the only thing I’d suggest is do what I did – I taught myself for years, and only now I’m taking lessons, but I can sight read – I used to write the notes down onto the piece (a photocopy normally), in pencil, then when I’d memorised how it was, I would play it without. it takes a lot of patience and practice. I’ve been playing for 10 years and my left hand is still shaky sight-reading.

what helps is to know note values right off the bat.
clapping rhythms helps a lot
look online and find pieces of music then clap the rhythm.
while you’re clapping the rhythm, move you hands up and down depending on what note your playing. (as in where the note falls in on the staff). this gives you a better idea on what note to hit while you’re singing
practice this a lot then find a completely new piece of music and try singing it. :]

Sight-singing is by far more difficult that sight reading with any other instrument. With the trumpet and especially the piano, you have a starting pitch. If you blow a Bb its going to come out at around a Bb. The same is not true for singing unless you have perfect pitch.
The best advice I can give is to become fluent in solfeg, i.e. do re mi fa sol la ti do. There are sight-singing books that give exercises to help train your body to sing intervals. The next step is practice.
