Learning to Read Music
Sheet music would tell you along with duration and rhythmic elements.
… Chord sheets do not.
Edit: Jack explains wonderfully the “music by osmosis principle” that so many have been trapped into.

How to Sight Read Music
Since you have not given an example of the music you are reading from, I will assume you are playing from a chord chart for guitar and you are playing a popular song and not something classical. You need to be able to hear the tune in your head while you are strumming the chords. Make sure you tap your foot and strum each time you tap This will work most of the time. As far as changing chords, you will need to hear in your head the chord changes.
